Health Declaration
I hereby declare that I am in good health and condition and I am physically able to participate in this walk. I hereby release and hold harmless and by this waive all claims for any injuries, death and invalidity to me which may be caused by any act or failure to act or the organizers, sponsors, members of Friends of the Environment directly or indirectly during the course of the walk or as a result of it.
Liability Release
I will not hold the organizers or their associates liable for any accidents, loss or damage as a consequence of participation in this Friends of the Environment sponsored walk.
Medical Services
I hereby release and forever discharge the organizer and sponsors of this walk, including but not limited to Friends of the Environment, from any claim whatsoever which arises or may hereafter arise on account of any first aid treatment or other medical services rendered in connection with my health status during my participation in the Friends of the Environment sponsored walk.
Media Release
I grant and convey to the organizers and sponsors of Friends of the Environment sponsored walk all rights, title and interests in all photographic images, videos or audio recordings of me or my likeness or voice made in connection with the walk by the organizers of the Friends of the Environment sponsored walk.
Volunteer Acknowledgement
I understand that this walk is a voluntary walk and no form of payment or compensation will be given to the walkers by the sponsors and/or organizers of this walk. I understand to read and abide by the code of conduct governing the walk which has been provided to me by organizers of Friends of the Environment.
